REQUIREMENTS :

A) RE-ISSUE OF A PASSPORT

1. Application Form printout, filled online https://mportal.passportindia.gov.in/mission/, to be submitted at the Embassy.

2. Photocopy of Passport, self-attested (Original passport to show)

3. Photocopy of Residence Permit and Work Permit, self-attested (Original to show)

4. Four recent colour photographs with white background, Size: 5cm x 5cm.

5. A letter from the company the applicant is working in (Click here)

B) LOST/DAMAGED PASSPORT

1. Application Form, filled online https://mportal.passportindia.gov.in/mission/

2. Photocopy of lost passport, self-attested

3. Photocopy of Residence Permit and Work Permit, self-attested (Original to show)

4. FIR from Police for lost passport along with English translation

5. Four recent colour photographs with white background, Size: 5cm x 5cm

6. Self-explanatory note explaining how the passport was lost

7. Company letter (Click here)

C) CHILD'S PASSPORT

If applying for fresh passport, start with step 1, otherwise start with step 2.

With effect from 15 January 2023, the service is available on SEWA portal:
https://indianconsularservices.mea.gov.in/consularServices/

a) Self sworn affidavit (Click here)
b) Self sworn Affidavit (NOC) (Click here)
c) Declaration from both parents that the child does not hold another passport
d) Proof of Indian citizenship of parents
e) Marriage certificate (original + copy with translation if applicable)

3. Photocopy of Passport, attested by parent

4. Photocopy of Residence Permit, attested by parent

5. Translated birth certificate + original copy

6. Parent's passport & residence/work permit (self-attested)

7. Four recent colour photographs (5cm x 5cm)

D) POLICE CLEARANCE CERTIFICATE

Issuance of PCC is subject to receipt of fresh Police Verification Report.
